Mounjaro® is Eli Lilly's branded tirzepatide — a once-weekly GLP-1 + GIP dual-agonist subcutaneous injection. It holds US FDA approval for chronic weight management and type 2 diabetes, and is dispensed in the Philippines under physician prescription per DOH AO 2020-0001 (telehealth provider model). Mounjaro® is administered as a once-weekly subcutaneous injection — into the thigh, abdomen, or upper arm. Each KwikPen is pre-filled by Eli Lilly at the dose your physician prescribes. Starting dose is typically 2.5mg/week, titrated upward over months based on your physician's protocol. Your Care Team checks in monthly via Viber.

In the SURMOUNT-1 trial (Jastreboff et al., NEJM 2022), adults with obesity or overweight without diabetes achieved an average body weight reduction of 15–22% over 72 weeks at therapeutic doses of branded tirzepatide. SURMOUNT-4 (Aronne et al., JAMA 2024) demonstrated that continued treatment maintained these losses. Individual results depend on dose, diet, sleep, and physical activity. Mounjaro® has a well-documented side effect profile from clinical trials. Most patients experience mild-to-moderate gastrointestinal effects, especially during dose titration:

• Nausea (most common; often improves within 2–4 weeks)
• Diarrhea or constipation
• Decreased appetite
• Vomiting (less common)
• Mild dyspepsia or abdominal pain
• Injection-site reactions

Less common but important to discuss with your physician:

• Acute pancreatitis (rare)
• Severe gastroparesis or ileus — seek medical attention if you experience persistent vomiting, inability to keep food down, or severe bloating
• Acute kidney injury — usually linked to severe dehydration from prolonged vomiting or diarrhea; maintain hydration
• Gallbladder or biliary issues
• Hypoglycemia risk if combined with insulin or other diabetes medications (sulfonylureas, meglitinides) — tell your physician all current medications
• Worsening of diabetic retinopathy — if you have diabetes, your physician will discuss whether eye monitoring is needed
• Serious allergic reactions (anaphylaxis, angioedema) — rare but life-threatening

US FDA boxed warning: tirzepatide is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN-2). Stop your dose and contact your physician immediately if you experience severe abdominal pain (especially upper-abdominal pain radiating to the back — possible pancreatitis), persistent vomiting, signs of dehydration (dark urine, dizziness, reduced urination), swelling of face/lips/throat, or any allergic reaction. Tell your physician about your full medical history before starting.

Medical emergency: if you experience difficulty breathing, swelling of face/lips/throat, severe chest or abdominal pain, or fainting, call PH emergency services (117) or go to the nearest emergency room — do not wait for a Viber response.

Branded Mounjaro® is the highest-cost option in our weight program because it is manufactured, packaged, and quality-controlled by Eli Lilly. Compounded Tirzepatide (₱3,999) contains the same active molecule prepared by a Philippine-licensed compounding pharmacy under physician order, and is appropriate for many patients — your physician will recommend the option suited to your assessment, budget, and medical history.

Mounjaro® is dispensed in the Philippines under physician prescription per DOH AO 2020-0001 (telehealth provider model); it is not yet registered with the Philippine FDA as a branded retail product. Use only under continuous physician supervision. Not for use during pregnancy, breastfeeding, in patients under 18, or with active pancreatitis or thyroid C-cell disease.
