Please read this document carefully before using LUMitalk's services. By completing a health assessment on our platform, you confirm that you have read, understood, and agreed to the terms set out below.
1. What is Telemedicine?
Telemedicine involves the delivery of healthcare services using electronic communications, where the patient and the healthcare provider are not in the same physical location. At LUMitalk, your health information is submitted through an online assessment form and reviewed by a licensed physician who determines whether a health program is appropriate for you.
2. How LUMitalk Works
LUMitalk is a digital health platform. We facilitate — we do not provide clinical services directly. Our role is to:
- Provide you with an online health assessment to capture your medical history and health goals
- Route your completed assessment to a PRC-licensed Filipino physician for independent clinical review
- If the physician determines a program is appropriate, coordinate the fulfillment of your program through licensed healthcare institutions and dispensing partners
- Support you through our care team throughout your program
All clinical decisions are made solely by independent licensed physicians. LUMitalk does not prescribe, store, or dispense any medication.
3. Benefits of Telemedicine
- Access to licensed physician review from the convenience of your home
- No need for an in-person clinic visit for initial health screening
- Coordinated access to licensed dispensing partners for delivery to your address
- Ongoing care team support via your preferred messaging channel
4. Limitations and Risks
You acknowledge and accept that telemedicine has inherent limitations, including:
- No physical examination. Our physician partners review your assessment remotely. They cannot physically examine you. Their determination is based entirely on the information you provide.
- Accuracy of your information. The quality of your clinical review depends on the accuracy and completeness of your responses. Inaccurate or incomplete information may affect clinical outcomes.
- Not a substitute for in-person care. Telemedicine complements but does not replace in-person medical care. If your condition requires physical examination or immediate attention, consult a physician in person.
- Technology limitations. Service delivery depends on platform and internet availability. Technical issues may affect your assessment or communications.
- Diagnostic limitations. LUMitalk's assessment is a health eligibility screening — not a comprehensive medical diagnosis.
- Program risks. All health programs, medications, and preparations carry potential risks and side effects that vary by individual. These will be addressed as part of the physician review process.
- Privacy risk. While we apply appropriate security measures, no electronic platform is completely immune from the risk of unauthorized access.
5. Health Programs
LUMitalk offers health and wellness programs across two categories: weight management programs and wellness and longevity programs. The specific programs available, including their compositions and applicable conditions, are described on the relevant program pages of our website.
All programs dispensed through LUMitalk are:
- Reviewed and approved by a PRC-licensed physician before fulfillment
- Prepared and dispensed by licensed healthcare institutions and compounding pharmacies pursuant to a valid physician order
- Subject to clinical eligibility criteria determined by the reviewing physician
All health programs and preparations carry potential risks and side effects that vary by individual health profile, medical history, and other factors. Results are not guaranteed and may vary. Your eligibility for any program may change if your health status changes — please inform our care team of any significant changes during your program.
The availability, composition, and terms of our programs may change over time. Current program details are always reflected on the relevant program pages of our website.
6. This Service is Not for Emergencies
LUMitalk is not an emergency service. If you are experiencing a medical emergency of any kind, do not use this platform. Call 911 or go immediately to the nearest emergency room or hospital.
7. Your Responsibilities
By using LUMitalk's services, you agree to:
- Provide complete, accurate, and truthful information in your health assessment
- Disclose all current medications, supplements, and known allergies or adverse reactions
- Disclose all pre-existing medical conditions relevant to your assessment
- Inform our care team if your health status changes significantly during your program
- Follow your program only as directed by the prescribing physician
- Seek in-person medical attention for any concern that cannot be addressed remotely
LUMitalk and its physician partners are not liable for adverse outcomes resulting from information that was inaccurate, incomplete, or withheld by the patient.
8. Data Privacy (Republic Act No. 10173 — Data Privacy Act of 2012)
What we collect
In connection with your use of LUMitalk's services, we collect personal information and sensitive personal information including your identification details, contact information, delivery address, health history, assessment responses, and program records.
Why we collect it
Your information is processed to facilitate physician review of your health assessment, fulfill your program order, coordinate with licensed dispensing partners, provide care team support, and comply with applicable Philippine laws and regulations.
Who we share it with
Your information may be shared with PRC-licensed physician partners for clinical review, licensed dispensing partners for fulfillment, licensed courier services for delivery, and regulatory authorities as required by law. We do not sell your personal information.
How long we keep it
Health and medical records are securely stored for the period required under applicable Philippine law and regulations, including those governing medical record retention. After the required retention period, records are securely destroyed.
Your rights
Under RA 10173, you have the right to be informed, access, correct, erase, object to, and seek portability of your personal data. You also have the right to lodge a complaint with the National Privacy Commission (NPC) at www.privacy.gov.ph.

9. Voluntary Consent and Withdrawal
Your use of LUMitalk's services is entirely voluntary. You may withdraw your consent at any time by contacting us at support@lumitalk.ph. Withdrawal of consent will mean we can no longer process new health information for program purposes. Records already created may be retained as required by our legal obligations.
10. Acknowledgment
By completing a health assessment on LUMitalk's platform, you confirm that you:
- Have read and understood this Telehealth Informed Consent
- Consent to the telemedicine assessment process, including asynchronous physician review
- Understand and accept the limitations and risks described above
- Acknowledge that LUMitalk is a platform facilitator and is neither a licensed clinic nor a licensed pharmacy
- Consent to the collection, processing, and sharing of your personal and health information as described in Section 8, in accordance with RA 10173
- Confirm that all information provided in your assessment is accurate and complete to the best of your knowledge
- Understand that this service is not for emergencies
